Prognostic Significance of Radiological Indices of Bone Density in the Maxillofacial Region for Choosing the Tactics of Using Orthodontic Appliances in the Treatment of Dentognathic Deformities in Children with Unilateral Congenital Cleft Lip and Palate

Over the past two decades, there has been a steady increase in demand for orthodontic treatment among both children and adults, primarily driven by patients’ desire to improve facial and smile aesthetics. Particular attention must be paid to the periodontal status of such patients, as the likelihood of periodontal pathology is relatively high. Epidemiological data indicate a significant prevalence of periodontal diseases, with severe forms of periodontitis diagnosed in 8–13% of the population. A clear correlation exists between periodontal disease and dentoalveolar anomalies (crowding, rotations, supra- and infraocclusion, etc.). These deformities complicate oral hygiene, promote plaque accumulation, and impair both aesthetics and functionality. The typical clinical profile of an adult patient with periodontitis often includes protrusion of the upper incisors, as well as the presence of tremas and diastemas.
Aim. To analyze the relationship between periodontal tissue condition and orthodontic anomalies in adults in order to improve their quality of life, and to investigate tissue response to tooth movement and treatment approaches.
Conclusions. It has been established that in patients with proper oral hygiene and healthy periodontal tissues, orthodontic intervention is safe for the alveolar bone. Even in the presence of periodontal disease, provided that hygiene is strictly controlled and moderate forces are applied (within the biological limits described by Schwarz), orthodontic treatment does not provoke exacerbation of inflammatory processes.
