Differentiated Application of Dental Materials in the Treatment with Fixed Dental Prostheses: A Literature Review

Abstract

The article reviews modern approaches to selecting materials and fixation systems for the manufacture of fixed orthopedic dental structures. It is shown that their reliability and durability depend not only on the material's properties but also on the type of cement and adhesive system. The physico-mechanical properties of metal-ceramics, ceramics, zirconia, composite materials, and metal alloys are analyzed. It was established that modified glass ionomer and composite adhesive cements provide stronger fixation than traditional cements. The importance of digital CAD/CAM technologies and pressing, which improve the fit of structures and reduce the risk of microleakage and complications, is demonstrated. The need for differentiated material selection and fixation systems to increase the effectiveness of orthopedic treatment and the durability of fixed structures is substantiated.
Objective: To analyze the scientific and practical works of domestic and foreign authors regarding the differentiated use of modern dental materials in the manufacture and fixation of fixed denture structures.
Results: A search for scientific publications in the PubMed, Scopus, Web of Science, and Google Scholar databases identified 428 sources potentially relevant to the research topic. After removing 112 duplicates, 316 publications remained for further analysis. During the initial screening of titles and abstracts, 198 works were excluded, leaving 118 articles selected for full-text analysis. After a detailed analysis of the content, another 72 publications were excluded for irrelevance to the topic or for lacking relevant results. Ultimately, 46 scientific articles were included in the systematic review. The analysis included original research, systematic reviews, and meta-analyses dedicated to the use of dental materials in fixed orthopedic prosthetics and the evaluation of their physico-mechanical or clinical properties. Full-text articles in Ukrainian and English were considered. Duplicates, conference abstracts, short communications, clinical cases, and publications without full text were excluded from the analysis.
Conclusions: The effectiveness of orthopedic treatment with fixed dentures largely depends on the selection of dental materials tailored to the various clinical stages. The physico-mechanical properties of materials, their adhesive characteristics, and dimensional stability affect the accuracy of structure fabrication, the quality of marginal fit, and the durability of restorations. The use of modern elastomeric impression materials and digital technologies increases the accuracy of anatomical structure transfer, while the use of adhesive composite cements ensures a stronger bond with tooth tissues and reduces the risk of microleakage.
